** To determine the right size for your child, please take the measurement based on the front side of one of his/her existing
    T-shirts in inches.
    For Width, please measure across the T-shirt from left to right, pit to pit.
    For Length, please measure from the shoulder, near the collar, down the length of the T-shirt.

I, Parent/Guardian, hereby give permission for my child/ward to attend Illumination Station KYdZ Camp 2026 from 26 Nov to 27 Nov and to participate in all related camp activities.

I understand that the teachers/persons-in-charge will do their best to ensure the safety and well being of my child/ward.

I will not hold the teachers or the church responsible for any injury, illness, accident or loss which may happen to my child/ward during the camp.

I also understand that my child/ward has to follow all instructions given by the teachers/persons-in-charge.

I consent to Kum Yan Methodist Church in taking photographs and videos of my child during KYdZ events for the publication of promotional materials relating to KYdZ Church.
